A Texas educator preparation program approved by the State Board for Educator Certification may use this form to purchase certification exam registration vouchers for their candidates. Each candidate may apply one voucher to their purchase of a Texas Educator Certification Examination registration. If the candidate tries to register with a voucher in a dollar amount that is not exactly equal to the full registration fee, the candidate will not be able to register.

Indicate how many vouchers you wish to purchase:

*NOTE: edTPA voucher prices are equal to the registration fee. EPPs will receive a discount equal to the amount that candidates receive as a rebate; this will be reflected in the invoice.

This form must be signed and dated below by the EPP legal authority. Vouchers will be sent via email to the above requester within two weeks after receipt of the completed form and payment or copy of the purchase order. Vouchers will be valid for a period of 12 months from the date they are generated.
